Insurance coverage for Spravato may be better than you expect.

PMC Psychedelic operates out-of-network with most PPO insurance plans. For many patients, this means little to no out-of-pocket cost for Spravato. We verify your coverage before your first visit, at no charge.

How being out-of-network works in your favor.

Most medical providers are in-network with insurance companies. That means they've agreed to accept lower, contracted rates — and in return, your insurance may cover more of the cost upfront.

PMC Psychedelic is entirely out-of-network. We have no contracts with any insurance company. This is a deliberate choice — and for Spravato patients with strong PPO coverage, it often works to their advantage.

  • We verify your specific benefits in advance

Out-of-network does not always mean better coverage. Some PPO plans have strong out-of-network benefits; others have weak ones. We verify exactly what your plan covers before recommending any treatment.

Which plans work with PMC.

We accept:

  • PPO (Preferred Provider Organization) plans — including most commercial PPO plans and many employer-sponsored PPO plans.
  • Union insurance plans with PPO out-of-network benefits — including many first responder, MTA, and municipal employee plans.
  • Out-of-network PPO coverage from major carriers — Aetna, Cigna, UnitedHealthcare, Anthem, BCBS, and others.

We do not accept:

  • HMO (Health Maintenance Organization) plans — these restrict patients to in-network providers.
  • Medicaid or Medicare at this time.
  • Plans with no out-of-network benefit.

Before your first appointment, our team contacts your insurance company directly to verify your out-of-network Spravato coverage. You will know what to expect before you commit to anything.

Ketamine IV infusion — out-of-pocket.

Ketamine IV infusion is not covered by insurance for mental health indications. It is paid out of pocket. We do not publish pricing on our website — cost details are discussed directly during your consultation.

We do not offer payment plans, packages, or financing arrangements for Ketamine IV at this time. If insurance coverage is a priority, Spravato may be the more appropriate option for eligible patients.

Common insurance questions.

What if my insurance denies coverage?

If your insurer denies coverage, we discuss alternative options with you — including whether Ketamine IV may be appropriate, or whether a different insurance plan might provide better coverage. We will not proceed with Spravato if coverage is denied without discussing the implications with you first.

Do I need a referral to come to PMC?

No referral is required. You can request a consult directly. We verify your insurance independently — no referral paperwork needed from your primary care provider.

Will Spravato show up on my Explanation of Benefits?

Yes. When insurance pays an out-of-network claim, it typically shows up on your Explanation of Benefits (EOB) statement. If privacy is a concern, discuss this with our team — we can walk you through what to expect.

Does the initial consultation cost anything?

The initial consultation fee varies based on your insurance coverage. For patients with PPO insurance, the consultation is often covered. For patients without applicable insurance coverage, a flat consultation fee applies. This is discussed before your appointment is confirmed.
